CBT Continuum Worksheet: Turn All-or-Nothing Thinking into a Measurable Range

A CBT continuum worksheet helps you examine all-or-nothing thinking by placing a specific behavior along a range. Use concrete examples to replace a broad label with a more precise judgment.

About this tool

A CBT continuum worksheet examines all-or-nothing thinking by placing a specific behavior along a range described in concrete terms. It helps you replace labels such as successful or unsuccessful with an account of what actually happened in a defined situation. Padesky’s 1994 paper describes continuum methods for core-belief work, including defining criteria that make a broad judgment easier to examine.

Use the same dimension from 0 to 100, with more of the chosen quality toward 100. Behavioral anchors describe what someone would do at several points, rather than who is a good or bad person. The ratings are personal estimates, not test scores. Keep the anchors steady when you re-rate, and do not treat the midpoint or the highest number as the required answer.

Completed example: After forgetting one document, Priya thought, “I am completely disorganized.” She chose preparation for routine meetings as her dimension and first placed herself at 15. Her anchors were 0: arrives without checking any materials; 25: checks the agenda but often misses needed items; 50: brings the main materials with some gaps; 75: brings materials and checks open questions; 100: checks every required item and prepares a backup for each known issue. In four recent meetings she brought the materials each time, missed one attachment, and reviewed open questions three times. She re-rated at 70 and concluded, “My preparation is usually useful, with a document-checking gap.” Her next step was a short attachment checklist.

A continuum describes a chosen behavior in a chosen context. It cannot measure your worth as a person. If the original belief combines unrelated qualities, such as competence and likability, use separate sheets so that moving along the line has a clear meaning.

Choose a dimension you can describe with actions. Being a good friend is difficult to judge consistently; following through on plans with friends is easier to examine. Keep the setting and period consistent, and include relevant circumstances such as illness or a changed agreement. If your anchors depend on guessing what other people secretly think, choose a behavior you can observe instead.

Use the completed worksheet to identify an action you can practice. For example, a conclusion about preparation might lead to checking attachments before sending a meeting invitation. The rating is an estimate tied to the anchors you wrote, so small differences between numbers do not establish meaningful psychological change. Explain your reasoning in words and keep the original sheet if you want to revisit that reasoning later.

Padesky’s paper describes clinical techniques through case examples; it does not validate this interactive worksheet as a diagnostic test or establish a success rate for using it alone. The exercise can support reflection within cognitive behavioral therapy, but its value cannot be judged by whether your revised rating rises. Evidence might reveal a strength, a limitation, or a question you still need to investigate.

Avoid using a continuum to minimize harm or excuse unsafe behavior. A specific harmful action can require boundaries or support regardless of where you place other behaviors on a line. If an exercise feels like pressure to dismiss your experience, pause. A licensed professional in Psychology.com's therapist directory can help you decide whether this approach fits the problem you are facing.

The Core Beliefs Worksheet can help identify a recurring judgment, while the Positive Data Log Worksheet records observations relevant to a balanced alternative over time. CBT Worksheets offers the broader collection. If anxiety is also affecting daily life, the Anxiety Test provides a separate screening check; a continuum rating itself cannot tell you whether you have an anxiety disorder.

CBT Continuum Worksheet FAQ

What should the two ends of the line mean?

Use less and more of one clearly defined quality. For meeting preparation, 0 might mean no preparation and 100 might mean every specified preparation step is done. Write the criteria before reviewing your evidence. Keep the same quality at each point so that your comparison remains understandable when you return to the worksheet.

Is this the same as rating how strongly I believe a thought?

No. The main line places a behavior or quality along a range. Belief conviction asks how certain you are that a sentence is true. This sheet uses the first kind of rating. You might still feel strongly attached to a harsh thought after recognizing that the observed behavior is more varied than the thought suggests.

Do I have to move my rating toward 50?

No. Keep your anchors and use the evidence, even if the number does not change. A useful outcome can be a more specific description of what needs attention. Describe the evidence that fits your position and choose a practical next step. The number is a personal estimate, without a pass or fail threshold.

Can I apply this to a broad label like failure?

First translate the label into one observable area, such as meeting deadlines during the past month. A single scale cannot capture a whole person. Specify what meeting a deadline would look like, include occasions when you followed through, and record the circumstances of missed deadlines. That gives you a behavior to understand or change.

Is the CBT continuum worksheet free to download as a PDF?

Yes. The CBT continuum worksheet is free to use, and you can download a PDF to keep your completed exercise. Include the anchors and the reasons for your rating so the result makes sense later. You can review it yourself or bring it to a conversation with a therapist.

Important: This is an educational practice tool, not a diagnostic assessment or a substitute for therapy. A qualified mental health professional can help you decide how to use it for your circumstances.
